Saturday, December 11, 12noon-2pm, this on-line event requires advance registration

Do you have questions about legally changing your name and or gender marker on your Ohio birth certificate? This is a free virtual legal clinic to help people who live in or outside of Ohio to change their name/gender marker on their Ohio birth certificates. Please be sure to include your county of birth in the intake paperwork (see below).

Join us for our free virtual legal clinic. You’ll be able to talk to a trained lawyer over the phone or through Zoom about the name and gender marker process on your Ohio birth certificate.

Step 1: Complete and submit the intake paperwork below.

Step 2: We will review your paperwork to make sure that it is complete and contact you with available clinic appointment dates and times.

Step 3: Respond to us with the appointment date and time that works for you.

Step 4: We will confirm your appointment on a “first come first serve” basis. We will send you a Zoom link or phone number for your appointment shortly before the clinic.

Intake paperwork is required at least 48 hours in advance of the virtual legal clinic. If the number of clients exceeds the capacity of the volunteer attorneys, the legal clinic coordinator will work to accommodate you at a future legal clinic!

Use this link to access the intake documents.

If you experience issues accessing the registration form, please email <legalclinics@equitashealth.com>; the legal clinic coordinator will reach out to find an accessible alternative.

This event is free and open to the public. If you are under 18 years of age, you will need a parent or guardian for the legal process. Bringing them to this clinic is not required but may be helpful.

Hosted by TransOhio and Equitas Health.
